This is not going to be a boring article about all the sexually transmitted diseases (STDs) you can get in Thailand, you can simply look that up on the internet, but a hands-on practical guide on how to manoever yourself through sex establshments and private contacts safely, based on real experience in Thailand.
The first thing you need to realize is, that although a sex worker (massage therapist, prostitute or freelancer) has been doing her job for years, has seen hundreds or probably thousands of customers and so more guys without underwear than you have seen girls out of their panties, there is still a chance that she doesn't know all about STDs and the risks.
You can notice this from simple things. For example, the clumsy way in which she puts on the condom, not being sure from which side to unfold it, or stretching it out wide and the putting it on your dick as if it's a lightbulb to be covered with a decoration. The belief that it's safe to give a customer a bare blowjob if it looks clean. And so on.
There are many girls working in the sex industry in Thailand with (minor) diseases but they think it's nothing.
So the point here is, that you got to take your own responsibility and YOU got to be the one to decide if something is safe or not, not her, despite her extensive experience.
Realize that raving reviews online about a business or about a girl don't show the reality that some guys actually have to be treated for STDs, because those guys won't be so proud to put that info there. They back off silently and so do the girls who are stopped from working.
While it is clear to everybody that you shouldn't have sex with her without a rubber on, there is still a lot of 'folk wisdom' about how safe it is for a working girl to give you a BBBJ (bare back blowjob). Of course it's great to still have some skin to skin contact, it's absolutely not without risk.
Surpisingly, a lot of working girls in Thailand believe that it is safe to give a bare blowjob, after they have seen 'how clean' it is. They believe that, by visual inspection during shower or during massage, they can diagnose if there are any diseases. However tempting it is, it's better to stay away from bare blowjobs by working girls. But if you want to take the, relatively small but still existing risk, you should choose wisely: your risk is the highest in erotic body-to-body massage parlours where the girls are forced or put under pressure to accept all customers for bare blowjobs. These are in all aspects the worst places to go to, and unfortunately there are some in Chiang Mai. If the girls are forced to give bare blowjobs to all customers, it means this is an unethical business and normally, if it's unethical in one way, it is unethical in many ways.
Girls who constantly give bare blowjobs to customers in erotic body-to-body massage parlours may have had the dick and sperm of the customer before you in her mouth just 15min before you arrive, and you will have sloppy seconds. This is the highest risk you can think of.
Girls in erotic body-to-body massage parlours who have the freedom to decide will now and then skip a penis when it looks or smells dirty, so that's already a big improvement.
Then there are the happy ending / teen massage parlours, where body to body massage or sex are not officially on the menu, but the girls negotiate the extras including full service by themselves. These are again better, because the girl won't constantly swallow dick.
Even better within the sex industry are girls who only massage and only after reluctant negotiation consent to take it in their mouth, this means that she rarely does it.
Of course the best is to have a regular girl, whether you support her or not, and make sure that she's not seeing other guys.
Blowjob bars are probably the worst places of all and luckily there aren't any in Chiang Mai.
As for cunnilingus, bare lickjobs, the same applies: you'd be a much higher risk licking a girl who is accepting full service all the time. Some of them will have HPV, which may not be considered a reason to stop working by the shop.
Just because a massage parlour or soapy is large, doesn't mean it's also reliable. Unfortunately, some large establishments in Chiang Mai with a high turnover are also unethical.
Besides that, if you go very cheap, you can't expect much. In the end, you get what you pay for. For example, going to the Burmese brothels in Santitam you can't expect these girls to be checked up regularly. They may or they may not. And the same is for sideline girls, there is no authority checking on them, they are on their own.
If you see or feel any symptoms, for example rash, red spots, warts or puss, obviously you go check with the doctor and there are ample hospitals in Thailand where you can check for STDs in a very affordable way. Some of the medicine could be cheaper to buy at a pharmacy, but they may not have the exact same medicine. Besides that, there are several clinics checking up for STDs as well.
And it's not a bad idea to check your blood regularly at a lab, this will be cheapest and you can actually check much more than STDs for a very affordable price. In fact, this is where some hospitals actually outsource their samples too, so it will be cheaper for you to go to those labs by yourself and you'll get the results faster.
In any way, don't panic. In most cases, nothing happens. Some guys already panic when a massage girl allowed you to finger her and he accidentally touched his dick later. There is no need to panic about it, because most working girls in established places are checked every 3 weeks or so. No need to loose sleep over it, but if you think you had a real exposure, just be a responsible person to yourself and others and check things a bit later.
When the girl uses nuru gel, this usually does not affect the silicon of the condom, but if the girl uses oil and there is lot of of oil on your dick, this may lead to tears in the silicon. It's not uncommon in Thailand to have the experience that a condom breaks, so make sure to wipe your dick clean of most of the oil and use good international brands. Some inferior brands tend to break as well, especially if you're larger sized than the Asian size the condom was designed for.
